About the nature of the relationship, maybe he has not really paid attention to what his wife wants at the moment, and since he knows she is a clean freak, he decided to buy her a vacuum cleaner. However, as a wife, I would expect something more meaningful or appropiriate for an anniversary gift like jewelry, flowers, a spa massage or anything meaningful. In addition, what is he suggesting by giving her a vacuum cleaner? Is he suggesting that her role in the relationship is to keep the home clean and that her place is the kitchen? Gifts can talk by themselves and a vacuum cleaner may also suggest that the husband thinks that the house is dirty and that the wife should clean it up. Personally, I would not like to receive a vacuum cleaner as an anniversary gift because is not just any item on a weekend, it is an anniversary gift. An annivesary represents time that we have invested on each other and love that has been growing through a year, so obviously I would like the gidt to be special.
